Please select the appointment type that best suits your needs. If I need to, I can assess your eligibility for an ACC claim on the day and adjust your appointment type accordingly. If you already have an existing ACC claim please bring you ACC45 number to your appointment - this looks like 2 letters followed by 5 numbers ie AB12345 and should be on your original letter that came through the post. If you don't have this information you can call ACC and request it. If you need to make a new claim we can do that together on the day of your appointment providing your injury is eligible.
